My Patient Portal

Effective March 1, 2024, the Lane My Patient Portal for access to your medical records will transition to MyChart.

You will still be able to view your past health information acquired prior to March 1, 2024, in My Patient Portal.

All health information acquired after March 1, 2024, will only be available in MyChart.

All of the information in My Patient Portal comes from your Electronic Medical Record acquired prior to March 1, 2024.
