Medical Records

The Medical Records Department is open Monday – Friday, from 8:30 a.m. – 4 p.m., to assist patients with medical record requests (personal, legal, and insurance), birth certificates, privacy compliance issues, and release of information forms.

Medical Record Requests

At Lane, we offer an online records request that verifies your identity by asking for a photo of your driver’s license, which you can take via webcam or smart phone. Please use a recent version of Chrome, Firefox, or Safari.

Who Can Sign Up to get Health Information for a Patient?

  • The patient (Not the spouse)
  • Power of Attorney if the patient is not able to sign (Legal document must be provided)
  • Parent (If the patient is younger than age 18)
  • Parent and minor if the patient is 12 to 17 years of age and is getting psychiatric, alcohol, or drug treatment services
  • Legal guardian (Proof of guardianship document must be provided)
  • Representative of the estate of a patient who expired. A copy of the death certificate and a copy of the representative of the estate must be provided

 

How to Request Medical Records

Request your medical records with our new online tool, and your records will be delivered right to you - no need to go and pick them up. Choose from Electronic or Mail delivery.

To receive copies of your medical records, please complete an Authorization to Release Health Information form.

You can either download the form from the link above or obtain it by contacting Health Information Management at 225-658-4311 or mrinbox@lanermc.org.

Request Medical Records Through MyChart

Existing patients who have signed up for MyChart may log in to view medical records. If you recently had an appointment, please review your After-Visit Summary to find an activation code and instructions on how to access your MyChart account.

If you are unable to locate the activation code or need assistance, please visit our MyChart page at lanermc.org/mychart or email or call MyChart Patient Support at MyChartsupport@fmolhs.org, 225-765-5727, or Toll Free: 888-888-6608.

Medical records will be available through MyChart beginning with 3/1/2024 visits. From your MyChart Menu select “Sharing Hub” and select the appropriate box to share your health information with.

Patients and proxies can request records through their MyChart account for records starting 3/1/2024. Requests can only be for yourself, as the records are delivered through MyChart. Images and pathology slides cannot be requested in this manner. Any other requests must be made by completing an Authorization for Release of Health Information Form

 

If your relative has expired, you can ask for copies of their medical records if:

  1. You are the power of attorney for health care that goes beyond the patient’s death, or you are the administrator of the estate. You must provide a copy of the legal document and the death certificate.
  2. You are considered an authorized relative if you are the surviving spouse of the patient who has expired. If there is no surviving spouse, you are considered an authorized relative if you are the eldest child, a parent, sibling, or other relative that is named and defined in the Louisiana rules of interstate succession.

 

To request the medical records of a relative who has expired:

  1. Download a release authorization form to allow Lane to release your deceased relative’s information.
  2. Send the release authorization form, a certified copy of the administrator of the estate, and a certified copy of the death certificate either in person or by mail to:
